Overview Community Support Worker: Community Support for Families - 6602 Program/Location: Community Support for Families; Norwich, CT. PC#: 1171. Employment type: Full-time, 40 hours/wk. Schedule: Monday-Friday, 8:30 AM-5:00 PM; 2 evenings per week of availability as needed. Summary: Community Support Workers and Parent Navigators assist families through a child and family team process that develops care plans based on functional strengths and needs. The program coordinates care for children and adults referred from DCF\'s Differential Response System. The team works with families to meet basic needs, behavioral health, substance use, and/or linkage to community services. Duties & Responsibilities Assesses whole family needs including basic needs, therapeutic, medical, and other needs. Provides support and crisis management related to family needs. Links families with needed community support services, programs, and agencies; advocates for clients’ rights and access to services. Acts as liaison with DCF, hospitals, other community-based services, family members, and significant others. Processes appropriate requests for flex funding. Collaborates with Parent Navigators. Additional duties as assigned. Qualifications Education: A bachelor’s degree in a Human Services related field is preferred; an associate’s degree plus two years’ related experience may be considered in lieu of a bachelor’s degree; or a high school diploma with a minimum of 4 years’ related experience will be considered. Experience: Minimum of 2 years\' experience in a mental health related setting. Licensure/Certification/Registration: Valid driver’s license. Other Qualifications: Bilingual in Spanish and English preferred. Compensation & Benefits Compensation: 20.00/hour minimum, with increases commensurate with experience. Actual rates determined at time of offer; may include program/location differentials or sign-on incentives. Part-time pro-ration may apply based on hours worked.
