COMMUNITY SUPPORT WORKER

Join to apply for the COMMUNITY SUPPORT WORKER role at UNM Hospital . Receive 17% weekday night, 26% weekend night, and 15% weekend day shift differentials. Minimum offer: $22.49/hr. Maximum offer: $33.74/hr. Compensation for this role is based on a number of factors, including but not limited to experience, education, and other business and organizational considerations. Position Summary Coordinate and provide services and resources to individuals/families necessary to promote rehabilitation, recovery, and resiliency. Identify and address strengths to aid recovery and resiliency, and barriers that could impede development of skills needed for independent functioning in the community. Support individuals and families in crisis situations; provide personalized interventions to develop, facilitate, or enhance decision making. The patient care assignment may include children at risk or experiencing serious emotional, neurobiological, and/or behavioral disorders; adults with severe mental illness (SMI), individuals with chronic substance abuse, and/or individuals with co-occurring disorders. Ensure adherence to hospital and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult, and geriatric age groups. Detailed Responsibilities Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice. Work to eliminate barriers to access culturally competent medical services for all patients; assist patients to access community resources, financial assistance programs, and services needed. Collaborate with medical providers, multidisciplinary team members, and community resource providers to assist patients meet treatment goals. Facilitate and encourage development of skills in activities of daily living, interpersonal coping, socialization, and community functioning; include adaptation to home, school, work, recreation, and social environments; pro‑active self‑care, nutrition, and money management. Plan for professional growth related to professional goals based on self‑assessment, evaluation, and feedback; assume responsibility for acquiring knowledge and experiences to meet goals. Offer information and resources to provide patient information on specific mental condition; serve as a mentor to promote recovery and resiliency and instill hope; teach symptom monitoring, management, and relapse prevention skills. Provide information about illnesses; assist individual with medication knowledge, side effects, discuss medication concerns with provider; facilitate self‑motivational skills for medication regimen. Assist in development and coordination of the individual’s service plan based on strengths and goals; include recovery/resiliency management plan, crisis management plan, and if requested advanced directives concerning behavioral healthcare. Assist client in obtaining access to and coordination of rehabilitative, medical, and other services; include financial and medical benefits/entitlements; assist with safe, affordable, stable housing; support behavioral health consumers involved in the judicial system. Encourage development and succession of natural supports in workplace, school, family, and community environments. Work with the individual/family to identify strengths, needs, and barriers to achieving self‑identified goals; conduct ongoing assessments to determine if services accessed meet needs. Assess, support, and intervene in crisis situations; facilitate development and use of crisis management plans that recognize early signs of crisis/relapse and use natural supports; encourage alternatives to hospital emergency departments and inpatient services. Coordinate and monitor use of services; track client activities relative to care plan; review documentation; maintain contact with external providers; conduct post‑discharge follow‑up contact as appropriate. Document all client encounters; complete and submit billing documentation; maintain current and comprehensive client files. Maintain and report applicable statistics regarding programs and client services. Enhance professional growth through educational programs, literature, in‑services, meetings, and workshops. Follow patient safety‐related policies, procedures, and protocols. Demonstrate proactive approach to patient safety by seeking opportunities to improve processes. Identify and report/correct environmental conditions or situations posing undue risk. Report potential or actual safety concerns, medical errors, or near misses timely. Encourage patients to actively participate in their own care by asking questions and reporting situations that aren’t understood or seem wrong. Qualifications Education Essential: Bachelor's Degree. Essential education specialization: related discipline. Experience Essential: 1 year directly related experience. Credentials Essential: CPR for Healthcare/BLS Prov or Prof Rescuers within 30 days; Valid New Mexico Driver's License; UNM Vehicle Operator's Permit within 60 days. Nonessential: Certified Psychosocial Rehabilitation Practitioner; Licensed Alcohol and Drug Abuse Counselor or LASI eligible. Physical Conditions Light Work: Exert up to 20 pounds occasionally, up to 10 pounds frequently, or negligible force constantly. May require walking or standing to a significant degree; may require pushing/pulling of materials. Working Conditions Minor hazard physical risks, dirt, dust, fumes, noise.
